Quality program screen
The screen that runs before the pitch rather than after it.
A quality or clinical director has a portfolio slot to fill. The agent screens candidate programs, asks whether a site really is worse than its peers, and works out what a measure result is worth under a withhold.

The job
Which improvement program is worth the slot? Is the site that looks worst actually worse, or is it a small sample? Was last year’s before-and-after the intervention, or the mean coming back? What is the measure worth under the contract? These are statistical questions that usually get answered with a bar chart.

What the agent does
- Screens the candidates
- Runs proposed programs through an ordered routing before anybody builds the business case.
- Measures reliability
- How much of a measured rate is the provider and how much is the sample.
- Compares to peers
- How far apart providers are on one published quality measure.
- Untangles the denominator
- How much of a readmission rate’s movement is the survivor denominator moving.
- Values the result
- What a measure result is worth under a capitation withhold, and where a declared score sits against declared cut points.
- Cites the evidence
- One published effect size at a time, with interval, population and PMID. Randomized and observational kept apart.

What it does not do
- Credit an intervention with regression to the mean. It says what the cohort would have measured anyway, first.
- Blend randomized and observational evidence. Separate tools, separate answers.
- Rank sites on noise. If the sample cannot support the comparison, it says so.
